Wellness Premium Incentives
New Horizons Baking Company is so committed to your health and wellness that we will reduce your monthly premium contributions for meeting certain goals. Please complete and return both forms by October 31, 2025 to earn as many premium credits as possible for 2025!
If you are a non-tobacco user, please complete and return the non-tobacco user affidavit and provide to your HR Manager with your enrollment forms. If you are currently a tobacco user and would like the opportunity to earn the non-tobacco user rate, you may enroll in the WellWorks For You Tobacco Cessation e-Learning Series. Upon completion of the program, as verified by WellWorks, you will be eligible to receive the non-tobacco user rates.
Resources
All full-time employees and spouses covered on the New Horizons Baking Company medical plan are eligible to participate in the wellness initiative through WellWorks. Simply schedule an annual preventive exam with your Primary Care Physician, bring a copy of the Proof of Annual Physical Form, undergo a preventive health screening, and ask your physician to sign the form and then send to WellWorks. Upon completion of this process, Human Resources will receive a monthly report of those that have completed their physical.
In order for your participation in the program to be tracked, eligible participants must be registered under the New Horizons Baking Company Portal. Please follow the steps below to log into your Wellworks For You account or create an account if you do not have one.
Important Note: Your health plan is committed to helping you achieve your best health status. Rewards for participating in a wellness program are available to all employees. If you think you might be unable to meet a standard for a reward under this wellness program, you might qualify for an opportunity to earn the same reward by different means. Contact HR and we will work with you to find a wellness program with the same reward that is right for you in light of your health status.
WellWorks – Tobacco Cessation Program
New Horizons has partnered with WellWorks to provide our employees a convenient tobacco cessation program. The Wellworks For You Tobacco Cessation e-Learning Series is an interactive program to help participants quit smoking, tobacco, or nicotine use. This six-week, self-study program is designed to educate, inform, and inspire behavioral change. The program contains a pre- and post-module survey, intended to assess a readiness to change in an individual, and six (6) modules that each include a video and a quiz. Participants will receive information to create a personalized quit plan, as well as online resources, to support participants through each step of the quitting process.
