Life insurance is an important part of your financial security, especially if others depend on you for support. Accidental Death & Dismemberment (AD&D) insurance is designed to provide a benefit in the event of accidental death or dismemberment.
New Horizons Baking Company provides Basic Life and AD&D Insurance to all benefit eligible employees at no additional cost. The coverage amount is $50,000.
In the event of an accidental death, the benefit may double. Please see your Guardian certificate booklet for further details.
In the event of an accidental dismemberment, a benefit is provided up to a scheduled amount corresponding to the loss. Please see your Guardian certificate booklet for further details.

Supplemental Term Life and AD&D
New Horizons Baking Company provides employees with the opportunity to purchase additional Life/AD&D insurance. Please see the worksheet for complete pricing details. Premiums increase when moving from one age bracket to another at the beginning of each plan year. Your premiums for this plan are deducted on a post-tax baisis.
Benefit Amount
Employee may elect in Increments of $10,000; overall maximum election is $300,000.
Spouse may elect in Increments of $5,000; overall maximum election is $150,000.
Child election is a flat $10,000; overall maximum election is $10,000.
