Eligibility
Medical:
Full-time employees, working at least 30 hours per week, are eligible on the 60th day of employment upon completion of the application for coverage. Spouses and dependent children are eligible for coverage. Dependent children are eligible up to the end of the month in which they turn 26.
Dental and Vision:
Full-time employees, working at least 30 hours per week, are eligible on the 60th day of employment upon completion of the application for coverage. Spouses and dependent children are eligible for coverage. Dependent children are eligible up to the end of the month in which they turn age 26.
Life and Disability:
Full-time employees, working at least 30 hours per week, are eligible on the 60th day of employment upon completion of the application for coverage.
Making Changes
Generally, you can only change your benefit elections during the annual benefits enrollment period. However, you may be able to change some of your benefit elections upon the occurrence of certain change in status events, provided you properly notify New Horizons Baking Company. You have 30 days from the date of the Life Event to make changes to your benefits. If you do not make updates within the 30-day enrollment period, you will have to wait until the annual Open Enrollment period to make changes. There are no exceptions.
These changes in status Life Events may include:
- Your marriage
- Your divorce or legal separation
- Birth or adoption of an eligible child
- Death of your spouse or covered child
- Change in your spouse’s work status that affects his or her benefits
- Change in your work status that affects your benefits
- Change in residence or work site that affects your eligibility for coverage
- Change in your child’s eligibility for benefits
- Receiving Qualified Medical Child Support Order (QMCSO)
If you have a family status change, you must notify your HR Manager and complete the necessary forms within 30 days.
Notes:
Most of the benefits are taken out on a pre-tax basis, reducing your taxable income.
